Purpose: In recent years, the discourse of participatory governance has gained significant attention as a modern approach to managing health systems. The purpose of this study is to develop a conceptual model of participatory governance in Iran’s health system, with a particular emphasis on the role and position of civil institutions in strengthening social participation and improving the quality of public policy-making in the health sector.
Methodology: This research is qualitative in nature and follows a thematic analysis approach. The data were collected through semi-structured interviews with 15 experts in the fields of health, public policy, and civil society. The data were analyzed at three levels—basic codes, organizing themes, and global theme—and the thematic network was visualized using MAXQDA software.
Findings: The results of the analysis revealed that Iran’s health system faces multiple challenges, including institutional opacity, limited public participation, lack of structural accountability, intersectoral incoherence, financial constraints, and institutional resistance. Conversely, civil institutions possess significant capacities, such as network-building potential, educational capabilities, and social empowerment mechanisms, which together provide a foundation for realizing participatory governance. The overarching theme of the study, “Participatory Health Governance,” encompasses three major clusters: challenges, role performances, and capacity-building mechanisms.
Conclusion: Based on the findings, revising the governance structure of Iran’s health system through the design of participatory mechanisms, strengthening accountability frameworks, and establishing legal foundations for the active engagement of civil institutions is essential.
